School Support

We appreciate that sometimes you may need a little extra help. On completion of this form an email will go to the appropriate department at the school(s) that you have indicated so that they can get in touch to discuss the support you require.

Please remember to select from the drop down box the school or nursery your child attends.  If you cannot see it listed then please select other and add it to the comments box.

If you do not have a child at school we may still be able to assist so please give us as much information as you can and we will assess your case, you may ask your GP, Social Care Provider or other Support Service to contact us.  In this instance please complete the form or ask your support service to contact us on 01376 617998 where a member of the team will pass details on to the appropriate person; please note that volunteers in the Hub are not authorised to make spot decisions so, while we understand your difficulties, please do not ask them to

Please give as much detail as possible so we can assess your case; information may be required from the school to determine the level of need and length of time that you will require support for. This may include, but is not limited to details of the correspondence received from support agencies, evidence of universal credit, details of redundancy or new employment etc.

The information you provide will assist us to ensure you receive adequate support for you needs. Please note this provision will support you on a short-term basis, you will be encouraged to use the help and support offered to enable you to become self-sufficient again.
